He is survived by his wife, Lori Phillips; son, Blaiz Phillips; aunts & uncles, Kay & Robert Cook, Anita Jones & Bobby & Gail Tribble; mother & father-in-law, Janet & Jim Bentley; brother-in-law, Chris Bentley; and several cousins, nieces & nephews. Funeral services will be conducted Tuesday, May 10, 2011 at 11 a.m. at Woodbine Funeral Home, HICKORY CHAPEL, 5852 Nolensville Road by Wendell Byrd. Interment Mapleview Cemetery.
